I give my consent to be photographed by Community Partnership staff for use in promotional materials and/or photo archiving related to the Hope Connection event on November 16, 2011.
I do not give my consent to be photographed during the Hope Connection event. (When you arrive at the registration table the day of the event, please pick up and wear a sticker that will indicate your desire not to be photographed.)
*In the event that your desired area is full, Hope Connection staff may reassign you to a different area. You will be contacted regarding this change.
